传统甲状腺切除术与小切口甲状腺切除术在甲状腺结节治疗中的临床效果比较

摘要：

目的 观察小切口甲状腺切除术与传统甲状腺切除术在甲状腺结节临床治疗中的应用效果。方法 选择我院2014 年12 月至2015 年12 月收治的120 例甲状腺结节患者，随机分为对照组和观察组，每组60 例。对照组采用传统甲状腺切除术治疗，观察组采用小切口甲状腺切除术治疗。比较两组手术时间、切口长度、总出血量、引流量、住院时间等手术指标，观察两组治疗过程中不良反应发生情况及镇痛药物使用情况。结果 观察组的手术切口长度、手术时间及住院时间均显著短于对照组（P<0.05）；观察组的术中出血量、引流量显著少于对照组（P<0.05）；观察组的切口粘连、颈部疼痛、吞咽不适等不良反应发生率及镇痛药物使用率均显著低于对照组（P<0.05）。结论 采用小切口甲状腺切除术治疗甲状腺结节的临床效果显著，对患者损伤小，术中出血量少，住院时间短，患者恢复快，值得临床推广应用。

Clinical effect of thyroidectomy and mini-thyroidectomy in the treatment of thyroid nodule

ABSTRACT: Objective To observe the clinical effect of thyroidectomy and mini-thyroidectomy in the treatment of thyroid nodule. Methods One hundred cases of patients with thyroid nodules in our hospital from December 2014 to December 2016 were selected and randomly divided into control group and observation group, with 60 cases in each group. Patients in control group were treated with conventional thyroidectomy, and patients in observation group were treated with mini -thyroidectomy. The operation time, incision length, total blood loss, flow volume，length of hospital stay, occurrence of adverse reactions and the usage of analgesic drugs of the two groups were compared. Results The incision length, operation time and hospitalization time of the observation group were significantly shorter than those of the control group (P<0.05); the intraoperative blood loss, flow volume of the observation group were significantly less than those of the control group (P<0.05). The incidences of adhesion of the incision, neck pain, swallowing discomfort, and the usage of analgesic drugs of the observation group were significantly lower than those of the control group (P<0.05) . Conclusion Mini-thyroidectomy has remarkable clinical effect with small damage, less intraoperative blood loss and shorter hospitalization time in patients with thyroid nodule, and can promote patients` recover, so it is worthy of clinical application.
