腮腺Warthin瘤上皮成分恶变为黏液表皮样癌的临床病理观察

摘要：

目的 探讨Warthin瘤上皮成分恶变为黏液表皮样癌的临床病理特征、诊断及鉴别诊断。方法 分析Warthin瘤上皮成分恶变为黏液表皮样癌的病例1例，研究其临床病理形态学特点、免疫表型及组织化学染色特点。结果 患者男性，27岁，腮腺区无痛性肿物，肿物呈结节状，切面囊实性。镜下Warthin瘤瘤体内可见黏液表皮样癌结构，为低级别黏液表皮样癌，两者存在移形区。结论 Warthin瘤上皮成分恶变为黏液表皮样癌极为罕见，病理诊断主要依据组织学形态，明确的黏液表皮样癌形 态及二者上皮成分存在移行，分子改变需要进一步研究观察。

Clinical pathologic features observation of mucoepidermoid carcinoma arising in Warthin

tumour of the parotid gland

ABSTRACT: Objective To investigate the clinical pathologic features, diagnosis, differential diagnosis of mucoepidermoid carcinoma arising in Warthin tumor. Methods One case of mucoepidermoid carcinoma arising in Warthin tumor was investigated by light microscopy, immunohistochemical staining and histochemical staining. Results The one case was a 27 years old male, who had a painless mass. The mass was nodular, and the cut surface was cystic and solid. Microscopic examination revealed that the tumor was mainly composed of Warthin tumor, but some area showed the morphological patterns of low grade mucoepidermoid carcinoma. A transitional zone between Warthin tumor and mucoepidermoid carcinoma existed. Conclusion Mucoepidermoid carcinoma arising in Warthin tumor is extremely rare. The diagonosis of carcinoma arising in Warthin tumor depended on the histological features, especially relies on the transitional zone between the Warthin tumor and the mucoepidermoidcarcinoma. Further studies are needed to observe molecular changes.
