不同剂量的布洛芬对足月新生儿动脉导管未闭的超声研究

摘要:

目的 比较口服不同剂量的布洛芬对足月新生儿动脉导管闭合的影响。方法 选取2014年1月至2016年5月我院收治的经超声心动图检查确诊的动脉导管未闭足月新生儿61例。患儿被随机分为高剂量治疗组(n=33)和低剂量治疗组(n= 28)。高剂量治疗组的布洛芬首日剂量为20 mg/kg,次日及第3日后再分别给予10 mg/kg;低剂量治疗组首剂量为10 mg/kg,次日及第3日后再分别给予5 mg/kg。治疗4天后两组分别复查超声心动图,观察动脉导管的闭合情况并测量PDA肺动脉端内径。结果 治疗4 天后,高剂量治疗组中有20 例动脉导管闭合,低剂量治疗组中有12例动脉导管闭合,组间差异不显著(P>0.05)。高剂量治疗组PDA 肺动脉端平均内径从2.09 mm 降至0.82 mm(P<0.05),低剂量治疗组PDA 肺动脉端平均内 径从1.75 mm 降低为0.89 mm(P<0.05)。结论 不同剂量的布洛芬对足月新生儿动脉导管闭合率无明显差异,两者均可显著降低动脉导管肺动脉端内径。

    Ultrasound study of different doses of ibuprofen on full-term neonates PDA

    ABSTRACT: Objective To compare the effects of different doses of oral ibuprofen on closure of full-term neonates PDA. Methods From January 2014 to May 2016, 61 full-term neonates with PDA in our hospital confirmed by echocardiography were selected. Patients were randomly divided into the low doses group (n=28) and high doses group (n=33). The high doses group was given initially 20 mg/kg ibuprofen, and then 10 mg/kg on the second and third day respectively. And the low doses group was given initially 10 mg/kg ibuprofen, then 5 mg/kg doses on the second and the third day respectively. Follow-up echocardiography was conducted on the fourth day, the closure situation of DA was observed and the inside diameter of pulmonic end of PDA was measured in the two groups. Results Four days after treatment, 20 cases of neonates were found that DA closed in the high doses group, while 12 cases in the low doses group, the difference was not significant (P>0.05). 4 days after treatment, the inside diameter of PDA pulmonic end decreased from 2.09 mm to 0.82 mm in the high doses group (P<0.05), while it was decreased from 1.75 mm to 0.89 mm in the low doses group (P<0.05). Conclusion There was no signifiant difference in closure of the DA between the two groups, and both of them can significantly reduce the inside diameter of PDA pulmonic end.
