多层螺旋CT在急性胰腺炎诊断及病情分级中的应用

摘要：
目的 探讨多层螺旋CT 在急性胰腺炎诊断及病情分级中的应用情况。方法 选取我院收治的46例急性胰腺炎患者作为研究对象，采用Philips Brilliance 64层螺旋CT机进行诊断并对其病情进行分级，并观察其影像学表现。结果 46 例急性胰腺炎患者中，水肿型34 例（73.91%）、出血坏死型12 例（26.09%）；A 级0 例、B 级14 例、C 级13 例、D 级11 例、E 级8 例。12 例出血性坏死型中，C、D、E 型患者中分别出现数量不等的不同程度的出血。46 例中存活44 例（95.65%），死亡2 例（4.35%）。急性胰腺炎的主要临床特征体现为患者的胰腺存在明显肿大的现象，胰腺周围的脂肪层相对模糊，可见少量积液，CT 增强扫描后，胰腺部位可见斑点状坏死现象。结论 多层螺旋CT在急性胰腺炎诊断及病情分级中具有重要的应用价值，可将其作为临床诊疗工作的优选方案推广使用。

Application of multi-slice spiral CT in the diagnosis and classification of acute pancreatitis

ABSTRACT: Objective To explore the application of multi-slice spiral CT in the diagnosis and classification of acute pancreatitis. Methods Forty-six patients with acute pancreatitis treated in our hospital were enrolled as the study objects, Philips Brilliance 64 -slice spiral CT was used to diagnose and classify the patients`` state of illness and imaging performance was observed. Results In the 46 patients with acute pancreatitis, there were 34 cases (73.91%) of edematous, 12 cases (26.09%) of hemorrhagic necrosis; there were 0 cases of grade A, 14 cases of grade B, 13 cases of grade C, 11 cases of grade D, 8 cases of grade E. Of the 12 cases of hemorrhagic necrosis, C, D, E patients were varying in the numbers and degrees of bleeding. Of the 46 patients, 44 (95.65%) survived and 2 died (4.35%). The main clinical features of acute pancreatitis is obvious pancreatic enlargement, with relatively blurred fat layer around the pancreas and a small amount of fluid, and the pancreas can be seen spotted necrosis after CT enhanced scan. Conclusion Multi-slice spiral CT has important value in the diagnosis and classification of acute pancreatitis, and it can be used as a preferred scheme for clinical diagnosis and treatment for acute pancreatitis.
