喉罩通气与气管插管全麻在老年患者腹腔镜胆囊切除术中的效果比较

摘要：

目的 观察喉罩通气全麻与气管插管全麻在老年腹腔镜胆囊切除术中的麻醉效果。方法 80例行腹腔镜胆囊切除术的老年患者随机分为对照组和观察组，每组40例。对照组患者采用气管插管全身麻醉，观察组患者采用喉罩通气全身麻醉，比较两组患者不同麻醉阶段的心率（HR）、平均动脉压（MAP），麻醉效果及麻醉相关并发症发生情况。结果 观察组患者麻醉诱导后、插管/罩后、气腹后、插管/罩后的HR、MAP显著低于对照组（P<0.05）。观察组患者的麻醉起效时间、阻滞完全时间、术后睁眼时间、自主呼吸恢复时间、定向力恢复时间显著优于对照组（P<0.05）。两组患者的麻醉相关并发症发生率比较，差异无统计学意义（P>0.05）。结论 喉罩通气全麻在老年腹腔镜胆囊切除术中的麻醉效果优于气管插管全麻，值得临床推广应用。

Effect comparison of the laryngeal mask airway and tracheal intubation general anesthesia in elderly patients with laparoscopic cholecystectomy

ABSTRACT: Objective To observe the effect of the laryngeal mask airway and tracheal intubation general anesthesia in elderly patients with laparoscopic cholecystectomy. Methods A total of 80 cases of elderly patients underwent laparoscopic cholecystectomy were randomly divided into control group and observation group, the control group was given tracheal intubation general anesthesia, the observation group was given general anesthesia with laryngeal mask airway. The heart rate (HR), mean arterial pressure (MAP) at different stages of anesthesia, anesthetic effect and complications of anesthesia were compared between the two groups. Results The HR and MAP of the observation group after anesthesia induction, intubation/mask, pneumoperitoneum and intubation/mask cover were significantly lower than those of the control group (P<0.05). The anesthesia onset time, anesthesia block time, the times of postoperation eye opening, spontaneous breathing recovery and directional force recovery in the observation group were better than those in the control group (P<0.05). There was no significant difference in the incidence of anesthesia complication between the two groups (P>0.05). Conclusion The effect of laryngeal mask airway anesthesia in elderly patients undergoing laparoscopic cholecystectomy is better than that of general anesthesia by tracheal intubation, which is worthy of clinical application.
