微生物检验质量的影响因素与病原菌耐药性研究

摘要:

目的 分析影响微生物检验(ME)质量的主要因素以及病原菌(Pathogenic Bacteria, PB)的耐药性情况,为细菌感染性疾 病的治疗提供参考。方法 选取我院2014年7月至2015年7 月行ME 的560例患者进行研究,分析不同菌株的数量及检测 质量的影响因素。结果 ME 质量的影响因素包括检验人员操作能力、标本采集与保存质量、检测方法;分离出的PB 共603 株,革兰氏阴性菌最多,为312株(51.74%),其次为革兰氏阳性菌240 株(39.80%)、真菌51株(8.46%);大肠埃希菌对于喹诺酮类药物、头孢类药物耐药率超过50.00%,金黄色葡萄球菌对于喹诺酮类、红霉素类和茁-内酰胺类药物的耐药性超过 50.00%。结论 了解ME质量的影响因素对制定针对性的干预方式提升检测效果具有重要意义,通过耐药性实验研究能够为临床治疗过程中抗生素药物种类的选择提供依据。

    Study on the influencing factors of the microbiological examination quality and drug resistance of pathogenic bacteria

    ABSTRACT: Objective To analyze the influencing factors of the laboratory microbiological examination (ME) quality and drug resistance of pathogenic bacteria (PB), to provide the reference for treatment of bacterial infection disease. Methods A total of 560 cases of patients with ME in our hospital from July 2014 to July 2015 were selected, and the number of different strains and the influencing factors of ME quality were analyzed. Results The factors affecting the quality of ME include the quality of the equipment, the ability of inspection personnel, the collection and storage quality of specimen, the detection method. There were 603 PB were isolated, the largest proportion of Gram -negative bacteria was 312 strains (51.74%), followed by Gram-positive bacteria of 240 strains (39.80%), fungus 51 strains (8.46%); the resistance rate of escherichia coli to quinolones, cephalosporins were more than 50.00% , the resistance rate of staphylococcus aureus to quinolones, erythromycin and 茁-lactam were more than 50.00%. Conclusion It is important to understand the influencing factors of ME quality for development of the targeted intervention and improvement of the detection effect; the study of drug resistance can provide the basis for the selection of antibiotics in the course of clinical treatment.
