新生儿肺出血停止后应用高频震荡通气治疗的 临床分析

摘要:

目的 研究及判定新生儿肺出血停止后应用高频震荡通气治疗的临床效果。方法 选取2014年2月至2016年1月本院收治的60例肺出血新生儿作为本次的研究对象,其中采取常规机械通气的30 例新生儿作为A 组,于肺出血体停止后应用 高频震荡通气治疗的新生儿作为B组,并观察两组新生儿的氧合指标情况、肺出血时间、上机时间、住院时间及氧疗时 间。结果 两组新生儿治疗前的OI,a/APO2 比较,无显著差异(P>0.05);治疗后,两组新生儿的OI 呈下降趋势,a/APO2 逐步升 高,且B 组新生儿1、6 及24 h 的OI,a/APO2 均明显优于A 组(P<0.05)。B 组新生儿的肺出血时间、上机时间、住院时间、氧疗 时间均短于A 组(P<0.05)。结论 新生儿肺出血停止后应用高频震荡通气治疗的临床效果显著,值得在临床中推广实施。

    Clinical effect of high freguency oscillatory ventilation after neonatal pulmonary hemorrhage

    ABSTRACT: Objective To study and determine the clinical effect of high freguency oscillatory ventilation after neonatal pulmonary hemorrhage. Methods Sixty neonates with pulmonary hemorrhage admitted in our hospital from February 2014 to January 2016 were selected as the study objects. Among them, 30 cases of neonates received conventional mechanical ventilation after pulmonary hemorrhage were treated as the A group and 30 cases of neonates accepted high freguency oscillatory ventilation were treated as B group. Then the oxygenation indexes, pulmonary hemorrhage time, ventilation time, hospitalization time and oxygen therapy time were observed in the two groups. Results There was no significant difference in the OI and a/APO2 between the two groups before treatment (P>0.05); after treatment, the OI showed a downward trend, a/APO2 gradually increased, and OI, a/APO2 at 1, 6 and 24 h in the B group were better than those of the A group (P<0.05). The pulmonary hemorrhage time, ventilation time, hospitalization time and oxygen therapy time in the B group were shorter than those of the A group (P<0.05). Conclusion High freguency oscillatory ventilation has obvious effect on neonates after neonatal pulmonary hemorrhage, which is worthy of promotion in clinic.
