超声乳化白内障吸除联合小梁切除术用于青光眼伴白内障的治疗效果分析

摘要：

目的 探究超声乳化白内障吸除联合小梁切术除治疗青光眼伴白内障的效果及安全性。方法 选择2015年2月至2016年10月本院收治的76 例青光眼伴白内障患者作为研究对象，随机分为对照组和观察组，各38 例。观察组行超声乳化白内障吸 除联合小梁切除术治疗，对照组行囊外白内障摘除联合小梁切除术治疗，观察两组患者的临床效果、并发症发生情况和角膜内皮细胞计数变化情况。结果 观察组患者的治疗总有效率为94.47%，显著高于对照组的76.32%（P<0.05）。观察组并发症总 发生率（10.53%）稍低于对照组（21.05%），但差异不显著（χ2=1.583，P>0.05）。术后，两组患者的角膜内皮细胞计数均先降低后 升高，且观察组术后1 个月的角膜内皮细胞计数低于对照组（P<0.05），但术后3个月的角膜内皮细胞计数稍低于对照组，差 异不显著（P>0.05）。结论 白内障合并青光眼采用超声乳化白内障吸除联合小梁切除术治疗效果较好，值得推广应用。

Effect of phacoemulsification combined with trabeculectomy on glaucoma complicated with cataract

ABSTRACT: Objective To study the efficacy and safety of phacoemulsif ication combined with trabeculectomy on glaucoma complicated with cataract. Methods From February 2015 to October 2016, 76 patients with glaucoma complicated with cataract in our hospital were randomly divided into control group and observation group, with 38 cases in each group. The observation group underwent phacoemulsification combined with trabeculectomy, while the control group received extracapsular cataract extraction combined with trabeculectomy. The clinical effects, complication rates and corneal endothelial cell count were compared in both groups. Results The total effective rate of the observation group was 94.47%, which was significantly higher than 76.32% of the control group (P<0.05). The total complication rate in the observation group (10.53%) was slightly lower than that in the control group (21.05%), but the difference was not significant (χ2=1.583, P>0.05). After surgery, the corneal endothelial cell counts in both groups were decreased first and then increased, the corneal endothelial cell counts in the observation group at 1 month after operation was lower than that in the control group (P<0.05), however, the number of corneal endothelial cells at 3 months postoperatively was slightly lower than that of the control group, the difference was not significant (P>0.05). Conclusion Phacoemulsification combined with trabeculectomy has good effect on cataract complicated with glaucoma, which is worthy of application.
