人工全髋关节置换术与半髋置换术在老年股骨颈骨折治疗中的疗效研究

摘要：

目的 分析人工全髋关节置换术与半髋关节置换术在老年股骨颈骨折治疗中的临床效果。方法 本次研究抽取2015 年 4 月至2016 年7 月入我院治疗的66 例老年股骨颈骨折患者为研究对象，依据临床不同治疗方法分为试验组和对照组，每组 33 例患者。其中，将接受人工全髋关节置换术的33 例患者设为试验组，接受半髋置换术的33 例患者设为对照组，观察对比 两组老年股骨颈骨折患者的手术用时、术中出血量、总输血量、住院天数及治疗后疼痛程度。结果 通过对老年股骨颈骨折患 者实施不同方法治疗，试验组患者手术用时为（139.37±22.13）min，同对照组的（105.59±33.20）min 比较，差异具有统计学意义（P<0.05）。两组患者的术中出血量、总输血量及住院天数比较，差异无统计学意义（P>0.05）。从术后髋关节疼痛缓解情况来 看，试验组患者无痛率（72.7%）明显高于对照组（48.5%），差异有统计学意义（P<0.05）。结论 在老年股骨颈骨折治疗中，人工 全髋关节置换术与半髋置换术各有优势，但是人工全髋关节置换术可以使患者的疼痛程度明显缓解，因此临床需结合患者 的实际情况进行选择。

Clinical effects study of artificial total hip arthroplasty and hemiarthroplasty in the treatment of elderly femoral neck fracture

ABSTRACT: Objective To analyze the clinical effects of artificial total hip arthro plasty and hemiarthroplasty in the treatment of elderly femoral neck fracture. Methods Sixty-six elderly patients with femoral neck fractures admitted in our hospital from April 2015 to July 2016 were enrolled in this study, and divided into experimental group and control group according to clinical treatment methods, with 33 cases in each group. Patients who underwent artificial total hip arthroplasty were selected as the experimental group, and patients who underwent hemiarthroplasty as the control group. The operative time, intraoperative blood loss, total blood transfusion, length of hospital stay and post-treatment pain were observed and compared between the two groups. Results Through the implementation of different treatment methods of elderly patients with femoral neck fracture, the operation time of patients in the experimental group was (139.37±22.13) min, which was significantly different from (105.59±33.20) min in the control group, the difference was statistically significant (P<0.05). There were no significant differences between the two groups in the amount of blood loss, total blood transfusion and length of hospital stay (P>0.05). From the postoperative hip pain relief situation, the pain relief rate (72.7%) in the experimental group was significantly higher than that in the control group (48.5%) (P<0.05). Conclusion In the treatment of elderly femoral neck fracture, artificial total hip arthroplasty and hemiarthroplasty both have their advantages, but artificial total hip arthroplasty can effectively alleviate the pain of patients, so it need to base on actual situation of patients when choose the treatment method in clinic.
