八珍汤对风湿性心脏病瓣膜置换术后患者红细胞的影响

摘要：

目的 研究传统中医药方剂八珍汤对风湿性心脏病瓣膜置换术后红细胞的影响。方法 将行风湿性心脏病二尖瓣置换 术+主动脉瓣置换术的患者30 例随机分为治疗组和对照组，各15 例。治疗组于手术前3 d 及术后5 d 连续服用八珍汤。观察 并比较两组患者体外循环（CPB）血中异形红细胞的百分比和红细胞形态；检测红细胞膜上Na+-K+-ATP 酶活性；比较两组术 后RBC、Hb 及MCH 水平。结果 治疗组CPB 中异形红细胞百分比低于对照组，红细胞形态明显优于对照组，Na+-K+-ATP 酶 活性高于对照组（P<0.05）。术后治疗组RBC、Hb、MCH 均高于对照组，差异有统计学意义（P<0.05）。结论 风湿性心脏病瓣膜 置换术围手术期使用八珍汤能对红细胞起到保护及促生长作用。

Influence of Bazhen decoction on erythrocyte after valve replacement of rheumatic heart disease

ABSTRACT: Objective To study the effect of traditional Chinese medicine prescription Bazhen decoction on erythrocytes after valve replacement of rheumatic heart disease. Methods Thirty patients with rheumatic heart disease who underwent mitral valve replacement and aortic valve replacement were randomly divided into treatment group and control group, with 15 cases in each group. The patients in the treatment group were given Bazhen decoction from 3 days before surgery to 5 days after surgery. The percentage of abnormal erythrocytes and the erythrocyte morphology in cardiopulmonary bypass (CPB) of the two groups were observed and compared; the activity of Na+-K+ ATPase on the erythrocyte membrane was detected; the postoperative red blood cell count, hemoglobin content and mean corpuscular hemoglobin of the two groups were compared. Results The percentage of abnormal erythrocytes in CPB of the treatment group was lower than that of the control group, and the erythrocyte morphology was superior to that of the control group; the activity of Na+-K+ ATPase was higher than that of the control group (P<0.05). Compared with the control group, the levels of RBC, Hb and MCH in the treatment group were significantly higher (P<0.05). Conclusion The use of Bazhen decoction in the perioperative period of rheumatic heart disease valve replacement can protect and promote the growth of red blood cells.
