紫绀型先天性心脏病颅脑CT 表现及分析

摘要:

目的 探讨紫绀型先天性心脏病(CCHD)颅脑CT 的表现并分析双侧大脑中动脉及上矢状窦CT值增高与血红蛋白(Hb) 浓度、红细胞计数(RBC)之间的关系。方法 收集并分析38 例紫绀型先天性心脏病患者(CCHD组)与45例正常健康人(正常组) 颅脑CT平扫图像,观察图像特点并测量双侧大脑中动脉及上矢状窦的CT值,结合72 h内Hb 浓度及RBC进行统计学分析。 结果 CCHD 组患者颅脑CT平扫静脉窦及脑血管密度相较于正常组呈不同程度均匀性增高,以双侧大脑中动脉和上矢状窦为著。双侧大脑中动脉CT值与Hb、RBC值呈正相关(P<0.001);矢状窦CT 值与Hb 浓度、RBC 值呈明显正相关(P<0.001)。CCHD 组 患者Hb 浓度、RBC、双侧大脑中动脉及上矢状窦CT 值均高于正常组(P<0.001)。结论 紫绀型先天性心脏病患者颅脑CT平扫静 脉窦及脑血管密度呈不同程度均匀性增高,以双侧大脑中动脉和上矢状窦为著,血液密度增高与Hb 浓度、RBC 值呈正相关。

    Analysis of cerebral CT in patients with cyanotic congenital heart disease

    ABSTRACT: Objective To analyze the cerebral CT imaging features of cyanotic congenital heart disease (CCHD) and study the relationship between the density of brain vessel (bilateral cerebral middle artery and superior sagittal sinus) on CT scan and hemoglobin (Hb) concentration, red blood cell (RBC) count. Methods Brain plain CT scans of 38 CCHD (CCHD group) and 45 normal adults (normal group) were collected and analyzed. The CT values of the bilateral middle cerebral artery and superior sagittal sinus were measured. Statistical analysis was performed along with the Hb concentration and RBC detemined within 72 hours. Results Compared with the normal group, the different degrees of uniformity increased of density of intracranial venous sinus and cerebral vascular was showed in cerebral CT scanning of CCHD group, the bilateral middle cerebral artery and superior sagittal sinus increased more significantly. An obvious parallel correlation existed between the CT value of the bilateral middle cerebral artery and Hb concentration, RBC (P<0.001), as well as between the CT value of superior sagittal sinus and Hb concentration, RBC (P<0.001). The Hb concentration, RBC, and CT value of bilateral middle cerebral artery and superior sagittal sinus of CCHU group were higher than those of normal group (P<0.001). Conclusion Different degrees of uniformity increased density of intracranial venous sinus and cerebral vascular is observed in cerebral CT scanning of CCHD, especially with markedly increase of the bilateral middle cerebral artery and superior sagittal sinus. The increasing of blood denisity is correlative with elevated Hb concentration and RBC.
