心脏超声在慢性心衰患者诊断及心脏功能评价中的应用

摘要：

目的 研究心脏超声在慢性心衰患者诊断及心脏功能评价中的应用。方法 以我院收治的110 例CHF 患者为观察 组，100 例体检健康者为对照组。检测两组研究对象的NT-proBNP 值和心衰超声参数，并分析NT-proBNP 值与HFEI 评分、 左室射血分数的相关性。结果 两组的超声诊断参数与NT-proBNP 值均存在明显差异。其中，观察组的NT-proBNP 值、HFEI 评分、左房内径、左室舒张末径明显高于对照组，并且NYHA 分级越高NT-proBNP 值越大，HFEI 评分、左房内径、左室舒张 末径值越高；观察组的左室射血分数低于对照组，且NYHA 分级越高其值越低。NT- proBNP 值与HFEI 评分存在正 相关性（r =0.612）；NT-proBNP 值与左室射血分数存在负相关性（r=-0.746）。结论 心脏超声诊断能够全面的反映慢性心衰 患者的心脏运行功能，可靠性高于传统诊断方式，在慢性心衰患者的诊断过程中应当大力推广心脏超声技术的应用。

Application of cardiac ultrasound in the diagnosis and evaluation of cardiac function in patients with chronic heart failure

ABSTRACT: Objective To study the application of cardiac ultrasound in the diagnosis and evaluation of cardiac function in patients with chronic heart failure. Methods A total of 110 CHF patients in our hospital were taken as the observation group and 100 healthy persons as the control group. The NT-proBNP values and ultrasonic diagnostic parameters were detected in the two groups, the correlations between the NT-proBNP value and HFEI, left ventricular ejection fraction were analyzed. Results There were obvious differences in the diagnostic parameters and NT-proBNP values between the two groups. The NT-proBNP value, HFEI, left atrial diameter, left ventricular ejection fraction in the observation group were significantly higher than those in the control group, and the higher the NYHA grade was, the higher the NT-proBNP value, HFEI, left atrial diameter and left ventricular ejection fraction were. The left ventricular ejection fraction of the observation group was lower than that of the control group, and the higher the NYHA grade was, the lower the value was. There was a positive correlation between NT-proBNP value and HFEI fraction (r=0.612); there was a negative correlation between NT-proBNP value and left ventricular ejection fraction (r=-0.746). Conclusion Cardiac ultrasound can comprehensively reflect the cardiac function of patients with chronic heart failure, and is more reliable than traditional diagnostic methods. The application of cardiac ultrasound should be greatly promoted.
