180例住院2型糖尿病患者降糖用药情况调查

摘要：

目的 了解住院2 型糖尿病（T2DM）患者降糖药物的用药现状和血糖控制情况，为临床合理用药提供借鉴。方法 选取我院 收治的T2DM 患者共180 例，对所有患者的降糖用药情况进行调查分析，内容包括患者基线资料、血糖水平、降糖药物类别。结果 180 例患者口服用药50 例（27.78%），其中包括例单药口服19 例（10.56%），31 例两种及以上联合用药（17.22%）；单用胰岛素 36 例（20.00%），胰岛素联合口服降糖药94 例（52.22%）。药物类型使用率从高到低依次为胰岛素、二甲双胍、阿卡波糖、格列美 脲、瑞格列奈。持续用药3 个月后，口服降糖药的HbA1c 水平及BMI 值均明显低于其他两种方式（P<0.05），HbA1c 控制达标率 为20.00%（36/180），口服降糖药达标率明显高于其他两种方式（P<0.05）。结论 T2DM 住院患者在采取降糖药物治疗时以胰岛素联 合口服降糖药为主，但患者血糖控制情况尚不理想，临床还需根据患者病程和血糖波动选择合理的用药方案，提高降糖效果。

Investigation on hypoglycemic medication usage situation in 180 inpatients with type 2 diabetes mellitus

ABSTRACT: Objective To understand the status of hypoglycemic medication usage situation and blood glucose control in patients with type 2 diabetes mellitus (T2DM)，and to provide reference for clinical rational drug use. Methods A total of 180 T2DM patients were selected. The hypoglycemic medication situation of all patients were investigated, including patients baseline data, blood sugar level, hypoglycemic drug categories. Results Of the 180 patients, 50 cases (27.78%) orally took hypoglycemic agentstreated, in which 19 cases (10.56%) with single oral agent, 31 cases (17.22%) with two or more combinations; 36 cases (20.00%) with insulin alone, 94 cases (52.22%) with insulin combined oral hypoglycemic agents. Drug usage in descending order were insulin, metformin, acarbose, glimepiride, repaglinide. After 3 months of continuous medication, the HbA1c level and BMI of oral hypoglycemic agents were significantly lower than the other two methods (P<0.05); the target rate of HbA1c was 20.00% (36/180), the target rate of oral hypoglycemic agents was significantly higher than the other two methods (P <0.05). Conclusion T2DM hospitalized patients taking hypoglycemic drugs mainly is insulin combined oral hypoglycemic agents, however, the patients`` glycemic control is not ideal, the clinic should choose the reasonable drug program according to patients`` duration and blood glucose fluctuations, to improve the hypoglycemic effect.
