PFNA固定与人工股骨头置换治疗老年股骨转子间骨折的疗效比较

摘要：

目的 探析股骨近端防旋髓内钉（PFNA）内固定与人工股骨头置换治疗老年股骨转子间骨折的疗效。方法 选取我院收治的老年股骨转子间骨折患者80 例，按治疗方式不同分为对照组和试验组，各40 例。对照组采用PFNA 内固定技术，试验组应用人工股骨头置换术进行治疗，对比两组患者的临床疗效、术中出血量、手术时间、住院费用及关节功能的恢复程度。结果 治疗后，试验组患者的治疗效果和关节功能的恢复程度均优于对照组，且术中出血量、手术时间及住院费用均显著低于对照组（P<0.05）。结论 针对老年股骨转子间骨折患者，应用PFNA内固定治疗的效果显著优于人工股骨头置换术治疗，不仅可减少术中出血量，减轻患者疼痛，而且可促进关节功能的尽快康复，提高了生活质量，值得推广。

Comparison of the efficacy of PFNA fixation and artificial femoral head replacement

in the treatment of intertrochanteric fractures in the elderly

ABSTRACT: Objective To analyze the efficacy of PFNA fixation and artificial femoral head replacement in the treatment of intertrochanteric fractures in the elderly. Methods Eighty elderly patients with intertrochanteric fractures in our hospital were selected and divided into control group and experimental group according to the different treatment method, with 40 cases in each group. The control group was treated with PFNA fixation and the experimental group was treated with artificial femoral head replacement. The clinical efficacy, intraoperative blood loss, operation time, cost of hospitalization and recovery of joint function were compared between the two groups. Results After treatment, the treatment effect and the degree of recovery of joint function in the experimental group were better than those in the control group; the bleeding volume, operation time and hospitalization cost in the experimental group were significantly lower than those in the control group (P<0.05). Conclusion For the elderly patients with intertrochanteric fractures, the effect of PFNA fixation was significantly better than artificial femoral head replacement. It not only can reduce the amount of intraoperative blood loss, reduce pain in patients, but also promote joint function rehabilitation as soon as possible, improve the quality of life, worthy of clinical popularization.
