陕西省消化内镜现状调查研究

摘要：

目的 综合分析陕西省各级医院的消化内镜使用情况，为提高陕西省消化内镜诊疗水平提供理论依据。方法 通过网络填表方式对陕西省各级医院消化内镜从业人员、设施与设备配备及诊疗项目开展进行问卷调查。结果 目前，陕西省消化内镜医生<1 000名，其中具有中高级职称的内镜医师在二、三级医院中超过66%；在调查的267家医院中97%的医院已开展胃镜检查，64%已开展肠镜检查，40%可进行内镜下治疗。小肠镜与胶囊内镜检查，EMR、ESD治疗技术等已在三级医院中普及；超声内镜下注射治疗、腹腔神经节阻滞、POEM、PEG等仅在三级医院开展。基层医院存在内镜室面积过小、胃镜数量不足、经规范培训的医师缺乏、消毒不严等问题。50.6%的内镜室负责人对其执业表示满意。结论 我省可开展的内镜诊疗项目较全面，但能全面开展的医院尚有限，内镜医疗人员数量不足，内镜室硬件设施配备不足，消毒工作有待规范。今后，将进一步加强内镜工作人员的规范化专业培训，强化内镜消毒意识，推动学术交流，全面提高陕西省内镜诊疗水平。

Investigation on the status of digestive endoscopic work in Shaanxi

ABSTRACT: Objective To analyze the status of digestive endoscopy related work in all hospitals in Shaanxi Province, so as to provide a theoretical basis for the improvement of endoscopic diagnosis and treatment level. Methods A questionnaire survey was carried out on the staff, facilities and equipment, the diagnosis and treatment of digestive endoscopy in all levels of hospitals in Shaanxi Province with an online form-filling approach. Results At present, there are about 1 000 digestive endoscopists in Shaanxi Province, with more than 66% of whom are senior endoscopists in secondary and tertiary hospitals. Among the surveyed 267 hospitals, 97% and 64% have launched gastroscopic and colonoscopic examination respectively, while 40% could conduct endoscopic therapy. The technology of small bowel endoscopy and capsule endoscopy, EMR and ESD has been popularized in grade III hospital. However, some advanced techniques, such as endoscopic ultrasound-guided injection therapy, celiac ganglion block and transoral endoscopic myotomy are only performed in tertiary hospitals. The main problems found from the survey are limited endoscopy spaces, insufficient number of endoscopic examination, shortage of physicians with standardized training and non-standard disinfection measures. About 50.6% of the surveyed endoscopists were satisfied with their current practice situation. Conclusion Hospitals in Shaanxi Province conduct a broad spectrum of endoscopic procedures. However, hospitals capable of performing a comprehensive endoscopic therapy are limited. In addition, the number of endoscopists and related facilities are inadequate, the disinfection procedure should be standardized. Based on this survey, in near future, we will expand the standardized professional training for endoscopists, intensify awareness about endoscopic disinfection, and launch more academic discussions, to fully improve the endoscopic diagnosis and treatment levels in Shaanxi Province.
