腰硬联合阻滞麻醉在老年股骨头置换术中的应用观察

摘要：

目的 观察与分析腰硬联合阻滞麻醉在老年股骨头置换术中的应用效果。方法 选择本院在2012年2月至2015年2月收治的120例老年股骨头置换术患者作为研究主体，将其随机分为A组与B组。A组患者65例，B组患者55例。A组患者实施腰硬联合阻滞麻醉，B组患者实施硬膜外麻醉。对比两组患者的麻醉效果、生命体征及不良反应发生情况。结果 A组的麻醉起效时间、术后苏醒时间均短于B组，麻醉维持时间长于B组（P<0.05）；A组患者中血压下降及心率下降发生率均低于B组患者（P<0.05）；A组无不良反应，B组有2例出现寒战，两组不良反应发生率比较，无明显差异（P>0.05）。结论腰硬联合阻滞麻醉用于老年股骨头置换术，具有较显著的麻醉效果，且安全性较高，可以在临床中普遍应用并大力推广。

Application of lumbar-epidural-combined anesthesia in elderly patient with femoral head replacement

ABSTRACT: Objective To observe and analyze the application effect of lumbar-epidural-combined anesthesia in elderly patient with femoral head replacement. Methods One hundred and twenty cases of elderly patients with femoral head replacement in our hospital from February 2012 to February 2015 were selected as the research subjects, which were divided into group A and group B. There were 65 cases in the group A and 55 cases in the group B. Patients in group A were treated with lumbar-epidural-combined anesthesia, while patients in group B were treated with epidural anesthesia. The effect of anesthesia, vital signs and adverse reactions were compared between the two groups. Results The onset time, duration of maintenance time and postoperative recovery time of the patients in group A were better than those of group B (P<0.05). The blood pressure, heart rate decreasing in group A were better than those of group B, the differences were significant (P<0.05). There were no adverse reactions in group A, while there were 2 cases of shiver in group B, and there was no statistical difference between the two groups (P>0.05). Conclusion Lumbar-epidural-combined anesthesia in elderly patient with femoral head replacement has significant anesthetic effect and high safety, which can be widely used in clinical practice and is worthy of promotion.
