血清脂蛋白相关磷脂酶A2水平与颈动脉内中膜厚度相关性研究

摘要:

目的 探讨血清脂蛋白相关磷脂酶A2(Lp-PLA2)水平与颈动脉内中膜厚度相关性。方法 选取2016年8月至2016年10月我院心内科住院患者179例,常规检测Lp-PLA2水平,并同时测定颈动脉内中膜厚度,以175 ng/mL为临界值分为对照组和Lp-PLA2升高组,分析两组患者的颈动脉内中膜厚度差异性。结果 Lp-PLA2 升高组空腹血糖、总胆固醇、甘油三酯、LDL-C及HCY明显高于对照组(P<0.05);Lp-PLA2升高组血清Lp-PLA2水平、双侧颈动脉内中膜厚度明显高于对照组 (P<0.01);Lp-PLA2 水平与颈动脉内中膜厚度呈正向直线关系(r=0.671,P<0.01);年龄、血压、血糖、总胆固醇、甘油三酯、 LDL-C、HCY、LP-PLA2均是影响颈动脉内中膜厚度的独立危险因素(P<0.01);校正传统危险因素后,Lp-PLA2仍是影响颈动脉内中膜厚度的独立危险因素(P<0.01)。结论 Lp-PLA2 水平可预测颈动脉内中膜厚度,可作为颈动脉斑块稳定性的预测指标。

    Correlationship study between serum lipoprotein associated phospholipase A2 and carotid intima media thickness

    ABSTRACT: Objective To investigate the correlation between serum lipoprotein associated phospholipase A2 (Lp-PLA2) and carotid intima media thickness. Methods One hundred and seventy-nine patients in the department of cardiology of our hospital from August 2016 to October 2016 were selected, and the level of patients` Lp-PLA2 and carotid intima media thickness were detected. With 175 ng/mL Lp-PLA2 as the critical value, they were divided into control group and high Lp-PLA2 level group. The difference of carotid intima media thickness of the two groups were analyzed. Results The levels of fasting blood glucose, total cholesterol, triglyceride, LDL-C and HCY in the high Lp-PLA2 level group was higher than those in the control group (P<0.05); the level of Lp-PLA2 and carotid intima media thickness in the high Lp-PLA2 level group were higher than those in the control group (P<0.05). There was a positive linear relationship between Lp-PLA2 level and carotid intima media thickness (r =0.671,P<0.01). The age, blood pressure, blood glucose, and the levels of total cholesterol, triglyceride, LDL-C, plasma homocysteine and LP-PLA2 were independent risk factors of carotid intima media thickness (P<0.01); after adjusting traditional risk factors, Lp-PLA2 was also an independent risk factor the carotid intima media thickness (P<0.01). Conclusion Carotid artery intima media thickness can be predicted by Lp-PLA2, which can be used as a marker for predicting the stability of carotid plaque.
