糖尿病肾病患者尿L-FABP的检测及其临床意义

摘要:

目的 探讨尿肝型脂肪酸结合蛋白(L-FABP)在糖尿病肾病(DN)患者尿液中的表达水平及临床意义。方法 收集2015年1月至2015年12月在我院肾脏内科确诊的2 型糖尿病患者80例为观察组,另选40例年龄、性别匹配的健康体检者为 对照组。应用酶联免疫吸附法(ELISA)检测其尿液蕴-FABP含量,同时检测血肌酐(Scr)、尿酸(UA)、总胆固醇(TC)、甘油三脂(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、糖化血红蛋白(HbA1c)等临床指标。结果与对照组 (NC)比较,2型糖尿病患者血UA、TC、TG、LDL-C、HbA1c及尿L-FABP水平明显较高,HDL-C水平明显较低,差异有统计学
意义(P<0.05);P-DM组、MAU组、C-DN组L-FABP及尿UACR水平,组间比较,差异有统计学意义(P<0.05);尿L-FABP与UACR呈正相关(r=0.394,P<0.05),与TC、TG、HDL-C、LDL-C、HbA1c、UA和Scr 无相关性(P>0.05)。结论 尿L-FABP可反映DN的病情进展情况,检测其水平对于DN的早期诊断及预后判断有重要意义。

    Detection of urinary L-FABP and its clinnical significance in patients with diabetic nephropathy

    ABSTRACT: Objective To study the level of urinary L-FABP in diabetic patients with different stages, and to explore the clinical significance in diabetic nephropathy. Methods Eighty patients diagnosed as diabetes in our hospital from January to December in 2015 were enrolled in the study. Forty healthy subjects were selected as control group. To obtain serum and urinary samples of studied objects, urinary L-FABP, Scr, UA, TC, TG,LDL-C, HDL-C and HbA1c were measured by ELISA and biochemical methods respectively. Results Compared with the NC group, the leveles of UA, TC, TG, LDL-C, HbA1c and urinary L -FABP of the patients with type 2 diabetic were obviouly higher, and the level of HDL -C was significantly lower, the differences were statistically significant (P<0.05). To compare the levels of L-FABP and urinary UACR in group P-DM, group MAU and group C-DN, the comparison of two groups had significant differences (P<0.05). There was positive correlation between L-FABP and UACR (r=0.394, P<0.05), while the L-FABP and TC, TG, HDL-C, LDL-C, HbA1c were not related (P>0.05). Conclusion As a novel biomarker, urinary L-FABP can be used to diagnose diabetic nephropathy and predict its progression.
