内镜介入治疗急性梗阻化脓性胆管炎的临床观察及对炎症因子的影响

摘要：

目的 探讨对急性梗阻化脓性胆管炎（AOSC）患者实施内镜介入治疗的效果。方法 将108例AOSC患者分为内镜组（内镜介入治疗）和手术组（传统开腹手术治疗），各54例。比较两组患者的临床效果及相关指标。结果 两组患者的治愈率比较， 差异不显著（P>0.05）；治疗后，两组患者的ALT、AST与GGT水平均较术前降低，ALB较术前升高（P<0.05）；治疗后，两组患者TNF-α、IL-4、IL-6及hs-CRP水平均较术前降低（P<0.05），且内镜组患者的TNF-α、IL-4、IL-6及hs-CRP水平均低于手术组（P<0.05）。结论 内镜介入治疗AOSC的临床效果显著，可有效减轻炎症反应。

Clinical observation of endoscopic interventional therapy on acute obstructive suppurative cholangitis and its effect on inflammatory cytokines

ABSTRACT: Objective To explore the effect of endoscopic interventional therapy on acute obstructive suppurative cholangitis (AOSC). Methods One hundred and eight patients with AOSC were divided into endoscopic group (endoscopic intervention) and operation group (traditional open surgery), with 54 cases in each group. The clinical effects and relevant indicators of the two groups were compared. Results There was no significant difference on cure rate between the two groups (P>0.05). After treatment, the levels of ALT, AST and GGT of the two groups were lower than those before operation, and the levels of ALB were higher than those before operation (P<0.05). After treatment, the levels of TNF-α, IL-4, IL-6 and hs-CRP of the two groups were lower than those before operation, and those of the endoscopic group were lower than the operation group (P<0.05). Conclusion The clinical effect of endoscopic intervention in the treatment of AOSC is significant, which can effectively reduce the inflammatory response.
