COPD合并慢性呼吸衰竭患者肺康复治疗中实施无创机械通气的临床效果

摘要：

目的 探讨在COPD合并慢性呼吸衰竭患者肺康复治疗中实施无创机械通气的临床效果。方法 选取在我院接受治疗的COPD合并慢性呼吸衰竭患者80例，根据入院编号将其进行分组，单号为试验组，双号为对照组，各40 例，对照组给予常规治疗，试验组在常规治疗基础上给予无创机械通气机进行治疗，比较两组患者治疗前、后的动脉血二氧化碳分压（PaCO₂）、血氧分压（PaO₂）及呼吸情况。结果 治疗前，两组患者的PaCO₂、PaO₂ 以及呼吸情况比较，差异均无统计学意义（P>0.05）；治疗后，两组患者的PaCO₂ 较治疗前降低，PaO₂ 较治疗前提高，呼吸频率较治疗前降低，且试验组患者各指标均优于对照组患 者（P<0.05）。结论 对COPD合并慢性呼吸衰竭的患者采用无创机械通气机进行治疗，其临床效果确切，有利于患者肺功能的 好转。

Clinical effect of noninvasive mechanical ventilation on pulmonary rehabilitation in COPD patients

with chronic respiratory failure

ABSTRACT: Objective To investigate the effect of noninvasive mechanica l ventilation on pulmonary rehabilitation in COPD patients with chronic respiratory failure. Methods Eighty cases of COPD patients with chronic respiratory failure in our hospital were selected, which were divided into two groups according to the admission number, patients with odd numbers were selected as experimental group, patients with double numbers were selected as control group, with 40 cases in each group. The control group was given routine treatment, and the experimental group received noninvasive mechanical ventilation on this basis. The arterial carbon dioxide partial pressure (PaCO₂), oxygen partial pressure (PaO₂) and respiratory condition were compared between the two groups before and after treatment. Results Before treatment, there were no significant differences in PaCO₂, PaO₂ and respiratory condition between the two groups (P>0.05). After treatment, the PaCO₂ decreased, the PaO₂ increased, and the respiratory frequency decreased in the two groups, and the above indicators in the experimental group were better than those in the control group (P<0.05). Conclusion Noninvasive mechanical ventilation on pulmonary rehabilitation in COPD patients with chronic respiratory failure has exact effect, which is conducive to improve the pulmonary function of patients.
