后路椎间盘镜手术与开放椎板开窗术治疗腰椎间盘突出的疗效比较

摘要：

目的 探讨后路椎间盘镜手术（MED）与开放椎板开窗术（OD）治疗腰椎间盘突出的临床效果。方法 使用随机数字表法将我院收治的80例腰椎间盘突出症患者分为MED 组和OD 组，各40例。MED组行后路椎间盘镜手术，OD组行开放椎板开 窗术。对比两组治疗效果、手术相关指标及术后并发症发生情况。结果 MED 组患者治疗总有效率为95.00%，明显高于OD 组 的80.00%（P<0.05）；MED组患者手术时间、术中出血量、切口长度、术后引流量、卧床时间均明显优于OD 组（P<0.05）；MED组患者术后并发症发生率为17.50%，对照组术后并发症发生率为22.50%，差异无统计学意义（P>0.05）。结论 MED 治疗腰椎间盘突出症的临床效果好，且不增加患者术后并发症的发生率。

Efficacy comparison of the microendoscopic discectomy and open laminectomy in the treatment of lumbar disc herniation

ABSTRACT: Objective To investigate the clinical effect of microendoscopic discectomy (MED) and open discectomy (OD) in the treatment of lumbar disc herniation. Methods Eighty cases of patients with lumbar disc herniation in our hospital were randomly divided into MED group and OD group, with 40 cases in each group. The MED group accepted microendoscopic discectomy, while the OD group received open discectomy. The therapeutic effect, surgical related indicators and postoperative complications between the two groups were compared. Results The total effect rate of the MED group was 95.00%, which was significantly higher than 80.00% of the OD group (P<0.05); the operation time, blood loss,length of incision, postoperative drainage time, time in bed in the MED group were significantly better than those in the OD group (P<0.05); the incidence of complications in the MED group was 17.50%, which was 22.50% in the OD group, the difference was not statistically significant in the two groups (P>0.05). Conclusion MED has a good clinical effect in the treatment of lumbar disc herniation, less trauma, and does not increase the incidence of postoperative complications.
