不同机械通气方式治疗重症新生儿呼吸窘迫综合征近远期效果观察

摘要:

目的 比较高频振荡通气和间歇指令通气对重症呼吸窘迫综合征新生儿的临床治疗价值。方法 选取我科收治的重症 呼吸窘迫综合征新生儿67 例,按照入院序号进行随机分配,分为对照组(33例)和观察组(34 例)。所有患儿均接受常规检查 和治疗,对照组患儿采用间歇指令通气法,观察组患儿则应用高频振荡通气,观察比较两组患儿的治疗效果。 观察组患 儿的并发症总发生率为5.88%,稍低于对照组的12.12%,但无显著差异(P>0.05)。观察组患儿的氧暴露时间和通气时间均短 于对照组(P<0.05)。随着治疗时间的延长,两组患儿的动脉血氧分压升高,二氧化碳分压降低;且观察组治疗后12、24 h 的 上述指标均优于对照组(P<0.05)。结论 相较于间歇指令通气法,高频振荡通气法治疗重症呼吸窘迫综合征新生儿的效果更为显著,能够大幅度改善患儿的血气值,明显缩短其通气时间和氧暴露时间,降低并发症的发生率,减轻患儿家长的经济负 担,有益于患儿生命健康安全和日后生活质量的保障。

    Long and short-term effects of different mechanical ventilation in the treatment of severe neonatal

    respiratory distress syndrome

    ABSTRACT: Objective To compare the value of high frequency oscillatory ventil ation and intermittent mandatory ventilation on severe neonatal respiratory distress syndrome. Methods Sixty -seven cases of severe neonatal respiratory distress syndrome patients admitted in our department were randomly divided into control group (33 cases) and observation group (34 cases) according to the admission number. All children were given routine examination and treatment, the control group received intermittent mandatory ventilation, while the observation group accepted high frequency oscillatory ventilation. The therapeutic effects of the two groups were observed and compared. Results The overall incidence of complications in the observation group was 5.88%, which was slightly lower than 12.12% of the control group, but the difference was not significant (P>0.05). The oxygen exposure time and ventilation time of the observation group were shorter than those of the control group (P<0.05). With the extension of the treatment time, the arterial oxygen partial pressure increased and carbon dioxide partial pressure decreased, and those of the observation group in 12, 24 h were better than the control group (P<0.05). Conclusion Compared with the intermittent instruction ventilation, high frequency oscillatory ventilation has better effect on severe neonatal respiratory distress syndrome, which can improve the blood gas value, shorten the ventilation time and oxygen exposure time, reduce the incidence of complications, reduce the parents`` economic burden, and it is beneficial to the neonatal life safety and quality of life in the future.
