经外周中心静脉置管术在婴幼儿肠外营养中的应用与护理

摘要：

目的 探讨经外周穿刺中心静脉置管（PICC）在婴幼儿肠外营养（TPN）中的应用与护理。方法 分析我院36例经PICC 行TPN 的患儿临床资料，总结临床治疗、护理方法和经验。结果 置管失败2例，腋静脉穿刺置管4例，头静穿刺脉置管1 例， 肘正中静脉穿刺置管4 例，贵要静脉穿刺置管25例。进行2次PICC 置管完成TPN 治疗3 例，PICC 留置时间最短5 d，最 长29 d，术后发生导管堵塞2例，漏液1 例，导管感染1 例。结论 PICC 在婴儿置管时的操作有一定难度，但可长时间留置导 管，并发症较少，符合婴幼儿静脉高营养的需求，应用前景广阔。

Application and nursing of peripherally inserted central catheter in parenteral nutrition of infants

ABSTRACT: Objective To research the curative effect and nursing of peripherally inserted central catheter (PICC) in parenteral nutrition of infants. Methods The clinical data of 36 cases of infants underwent TPN through PICC in our hospital was analyzed. The clinical treatment, nursing methods and experience were concluded. Results Two cases of catheterization failed, and the others succeeded. There were 4 cases through axillary vein, 1 case through scalp vein, 4 cases through median cubital vein, and 25 cases through basilic vein. Three cases underwent 2 times of PICC. The shortest indwelling time of PICC was 5 d and the longest was 29 d. After operation, there were 2 cases with catheter blockage, 1 case with leakage and 1 case with catheter infections. Conclusion It is difficult to applicate the PICC in infant patients, but the complications were less and the tube can be indwelled for a long time, and this method conforms to higher intravenous nutrition of infant patients, which possesses wide application perspective.
