奥氮平与利培酮对精神分裂症患者血脂的影响研究

摘要：

目的 探讨奥氮平与利培酮对精神分裂症患者血脂的影响。方法 选择2015年1月至2015年12月在我院住院的122 例精神分裂症患者，按照入院顺序将患者分为奥氮平组61 例（单用奥氮平治疗）和利培酮组61 例（单用利培酮治疗），治疗8 周后，记录两组患者的血脂变化及精神症状情况。结果 两组患者的治疗总有效率比较，差异无统计学意义（P>0.05）。治疗8 周后，两组患者的PANSS 评分与治疗前比较，显著下降（P<0.05）；但两组治疗前及治疗8 周后的减分值比较，无显著差异（P> 0.05）。治疗8 周后，奥氮平组患者的TC、TG、LDL-C 水平升高，HDL-C 水平降低（P<0.05）；利培酮组患者的TC、TG 水平升高（P<0.05）；奥氮平组患者的TG 水平高于利培酮组，HDL-C 水平低于利培酮组（P<0.05）。结论 奥氮平和利培酮治疗精神分裂 症疗效相当，治疗过程中都会引起甘油三酯升高，但奥氮平升高甘油三酯作用更强，临床中应根据患者的实际情况合理选择 药物。

Effects of olanzapine and risperidone on blood lipids in patients with schizophrenia

ABSTRACT: Objective To explore the influence of olanzapine and risperidone on blood lipid in the treatment of schizophrenia. Methods From January 2015 to December 2015, 122 cases of patients with schizophrenia hospitalized in our hospital were selected, and all the patients were divided into olanzapine group (receiving olanzapine treatment alone) and risperidone group (receiving risperidone treatment alone) according to the admission order. After 8 weeks of treatment, the changes of blood lipid and psychiatric symptoms were recorded in both groups. Results There was no significant difference in the total effective rate of treatment between the two groups (P>0.05). After 8 weeks of treatment, the PANSS scores of both groups decreased significantly compared with those before treatment (P<0.05)；however, there was no significant difference in the decreased score before treatment and 8 weeks after treatment between the two groups (P>0.05). After 8 weeks of treatment, the levels of TC, TG, LDL-C increased and the level of HDL-C decreased in the olanzapine group (P< 0.05); the levels of TC and TG increased in the risperidone group (P<0.05); the level of TG in the olanzapine group was higher than that in the risperidone group, and the level of HDL-C was lower than that in the risperidone group (P<0.05). Conclusion Olanzapine and risperidone have equel therapeutic effect in the treatment of schizophrenia, and both can make the triglyceride level increase, but olanzapine has high increase triglyceride effect, the rational drug basesd on the actual situation of patients should be chosen in clinic.
