门诊西药房处方调剂差错分析及防范效果

摘要：

目的分析门诊西药房处方调剂差错的原因并提出相应的防范措施。方法 选取我院2014年1月至2017年1月门诊西 药房的处方调剂差错84例，经分类统计和回归性分析后总结门诊西药房处方调剂差错原因，并提出相应防范措施。结果 门诊西药房处方调剂差错的类型为药品名称差错7例（8.33%），药品规格差错3例（3.57%），药品数量差错27 例（32.14%），药品剂量差错12 例（14.29%），药品品种差错11例（13.10%），用药禁忌差错5 例（5.95%），药品分发差错19例（22.62%）。门诊西药房处方调剂差错原因包括药师原因（14.29%），医师原因（25.00%），患者原因（34.52%），环境原因（9.52%）和其他原因（16.67%）。结论 根据往年门诊西药房处方调剂差错的分析结果，提出并实施针对性的防范措施可有效提高处方药品调剂工 作质量，降低差错率，保证患者的用药安全。

Error analysis and preventive effect of prescriptions adjustment in outpatient dispensary

ABSTRACT: Objective To analyze the causes of errors and preventive effect of prescriptions adjustment in outpatient dispensary and provide some preventive suggestions. Methods A total of 84 cases of prescription errors in our hospital from January 2014 to January 2017 in outpatient pharmacy were selected. The reasons of prescription errors were analyzed by the classification statistics and regression analysis, and the corresponding preventive measures were putted forward. Results The type of prescription errors in outpatient pharmacy of our hospital included 7 cases of the drug name error (8.33%), 3 cases of drug specifications error (3.57%), 27 cases of drug amount error (32.14%), 12 cases of drug dose error (14.29%), 11 cases of drug varieties error (13.10%), 5 cases of contraindication error (5.95%), 19 cases of drug dispensing error (22.62% ). The reasons of prescription errors in outpatient pharmacy included the causes from pharmacists (14.29% ), physicians (25.00%), patients (34.52%), environmental factors (9.52%) and other reasons (16.67%). Conclusion According to analyze in outpatient pharmacy the reason of prescription errors in former years, the targeted preventive measures were putted forward, which can effectively increase the quality of prescription drug dispensing, reduce errors rate and ensure the safety of the patients.
