阑尾炎患者经小切口阑尾炎切除术治疗的临床效果观察

摘要：

目的 探讨小切口阑尾切除术治疗阑尾炎的临床效果。方法 择取我院2015 年4 月至2016 年4 月接收的阑尾炎患者 35 例作为研究对象，按数字奇偶法将其分为对照组（n=17）和治疗组（n=18）。对照组给予传统阑尾开腹手术，治疗组给予小 切口阑尾炎切除术。比较两组患者手术中情况（手术时间、术中出血量、切口长度）、术后情况（术后肛门首次排气时间、术后 恢复时间、首次下床活动时间）、术后各时间段VAS 评分及并发症发生情况。结果 治疗组患者切口长度和手术时间均明显短 于对照组，且术中出血量明显少于对照组，差异具有统计学意义（P<0.05）；治疗组患者术后肛门首次排气时间、术后恢复时 间及首次下床活动时间均明显短于对照组，差异具有统计学意义（P<0.05）；治疗组患者术后5 min、12 h、24 h 各时间段VAS 评分均低于对照组，差异具有统计学意义（P<0.05）；治疗组患者并发症总发生率为5.56%，对照组并发症总发生率为23.52%，治 疗组并发症总发生率显著低于对照组，差异具有统计学意义（P<0.05）。结论 对阑尾炎患者应用小切口阑尾炎切除术，疗效 显著，术后并发症较少，缓解疼痛，值得临床应用与推广。

Clinical effect observation of small incision appendectomy for appendicitis

ABSTRACT: Objective To investigate the clinical effect of small incision appendectomy for appendicitis. Methods From April 2015 to April 2016, 35 cases of appendicitis patients in our hospital were selected as the research objects and divided into control group (n=17) and treatment group (n=18) according to the number parity method. The control group was given conventional laparotomy appendectomy, and the treatment group was treated with small incision appendectomy. The condition during operation (operation time, intraoperative blood loss, incision length), the condition after operation (postoperative anal exhaust time, recovery time, the first time of getting out-of-bed), postoperative VAS scores at each period and incidence of complications were compared between the two groups. Results The length incision and operation time in the treatment group were shorter than those in the control group, and the amount of bleeding was less than that of the control group, and the differences were statistically significant (P<0.05). The postoperative anal exhaust time, recovery time, and the first time of getting out-of-bed in the treatment group were shorter than those in the control group, and the differences were statistically significant (P<0.05). The VAS scores at postoperative 5 min, 12 h and 24 h in the treatment group were lower than those in the control group, and the differences were statistically significant (P <0.05). The complication rate in the treatment group was 5.56%, which was significantly lower than 23.52% in the control group, and the difference was statistically significant (P<0.05). Conclusion The small incision appendectomy for appendicitis has remarkable therapeutic effect, reduce the incidence of complications, alleviate the pain, which is worthy of clinical application and promotion.
