螺旋CT 三维成像在颌面部复杂骨折中的应用

摘要:

目的 研究螺旋CT 三维成像在颌面部复杂骨折中的价值。方法 纳入我院收诊的70 例疑似颌面部复杂骨折患者,均经 常规CT 扫描和螺旋CT 三维成像检查,以手术病理结果为金标准,评价两种方法在颌面部复杂骨折诊断中的价值。结果 经手术病理证实,颧骨合并上颌骨骨折15 例,髁骨骨折9 例,粉碎性上下颌骨骨折10 例,筛骨骨折8 例,全面部骨折14 例,颧 骨颧弓骨折12 例,无骨折2 例。常规CT 扫描与螺旋CT 三维成像检出率均为100.00%,组间无显著性差异(P>0.05)。螺旋 CT 三维成像准确率为92.86%,略高于常规CT 扫描的82.86%(P>0.05)。结论 螺旋CT 三维成像应用于颌面部复杂骨折能清 晰显示骨折立体结构,对诊断骨折类型具有较高的准确性,但不能完全替代常规CT 二维扫描。

    Application of spiral CT three-dimensional imaging in complex maxillofacial fractures

    ABSTRACT: Objective To study the application value of spiral CT three-dimensional imaging in complex maxillofacial fractures. Methods Seventy cases of patients with suspected complex maxillofacial fractures diagnosed in our hospital were enrolled, and examined by routine CT scan and spiral CT three-dimensional imaging. The results of surgical pathology were selected as the gold standard to evaluate the value of two kinds of examination methods in the diagnosis of complex maxillofacial fractures. Results The surgical pathology confirmed that there were 15 cases of zygomatic maxillary fracture, 9 cases of condylar bone fracture, 10 cases of comminuted upper and lower jaw fractures, 8 cases of ethmoid fractures, 14 cases of panfacial fractures, 12 cases of zygomatic fracture and 2 cases of no fracture. The fracture detection rate of the conventional CT scan and spiral CT three-dimensional imaging were both 100.00%, there was no significant difference between the two methods (P>0.05). The accuracy of spiral CT 3D imaging was 92.86%, which was slighterly higher than 82.86% of the conventional CT scan (P>0.05). Conclusion The spiral CT 3D imaging can clearly show the three -dimensional structure of fractures in maxillofacial complex fractures, which has high accuracy in the diagnosis of fracture types, but can``t completely replace the conventional CT two-dimensional scanning.
