异常白细胞散点图对血细胞分析仪分类结果准确性的警示作用

摘要：

目的 通过对白细胞散点图异常标本的手工复检，探讨异常散点图对自动血细胞分析仪分类结果准确性的警示作用。方法 针对白细胞散点图细胞群落界限不明异常特征，分别选取38 例细胞群落分界清楚标本（A 组）及32 例细胞群落界限不明标 本（B 组）行手工分类复检，并与仪器结果作线性回归分析，分析其间相关性。结果 A 组标本手工复检结果与仪器分类结果相关 性较好，中性、嗜酸、淋巴细胞的r2 分别为0.9785、0.9667、0.9606，单核细胞次之（r2 为0.7344），嗜碱性细胞欠佳（r2 为0.3190）；B 组标本手工复检结果与仪器分类结果间的相关性较A 组降低。结论 异常白细胞散点图是独立于仪器其他报警信息之外的有 效警示信号，散点图出现细胞群落间界限不明、粘连融合异常特征时，需结合手工复检，确保检验结果准确性。

Warning effect of abnormal leukocyte scatter diagram on accuracy of hematology analyzer classification results

ABSTRACT: Objective To investigate the effect of abnormal scatter diagram on the accuracy of automatic blood cell analyzer by manual examination of abnormal spots of WBC. Methods In view of the abnormal characteristics of the WBC scatter plot and the unknown limit of the cell community, 38 cases of specimens with clear demarcated of the cell community (group A) and 32 cases of undefined cell community boundaries (group B) were given manual classification reexamination. Linear regression analysis was made between the results of manual classification reexamination and the instrument, and the correlation was analyzed. Results The correlations between manual classification reexamination and the instrument in group A were well, the correlation coefficient of Neu, Eos, Lym were 0.9785, 0.9667 and 0.9606, which in Mon was 0.7344, which in basophilic cells was 0.3190. The correlations between manual classification reexamination and the instrument in group B were lower than group A. Conclusion The abnormal leukocyte scatter diagram is an effective warning signal independent of other alarm information of the instrument. When there is no boundary between cells in the scatter plot and the abnormal characteristics of adhesion and fusion, the manual examination should be combined to ensure the accuracy of the results.
