基层医院中药处方的常见问题及对策

摘要：

目的 对基层医院中药处方的常见问题及对策进行分析与探讨。方法 抽取我院2015年1月至2015年12月的中药处方共1200张，将每张处方的患者年龄、性别、疾病诊断、中药用药、剂数、中药处方帖数、药名书写、处方脚注、处方用药味数、处方单帖费用、配伍禁忌、用法等资料进行调查。结果 1200张处方中，涉及问题的共230 张，占比为19.2%，主要包括处方脚注、处方前记、诊断与用药不适宜、药名书写不当等。结论 中药处方的安全、有效是直接影响中药疗效的重要因素之一，也是中医临床用药安全、有效、经济的保障，不论是中医师还是中药调剂人员，都要依据病情审慎地筛选药物，决定剂量和用法，从而提高用药疗效，减少中药不良反应的发生。

The common problems and countermeasures of traditional Chinese medicine prescriptions in primary hospital

ABSTRACT: Objective To analyze and discuss the common problems and countermeasures of traditional Chinese medicine prescriptions in the basic level hospital. Methods A total of 1 200 prescriptions of traditional Chinese Medicine in our hospital from January 2015 to December 2015 were selected, then the date of age, sex, disease diagnosis, medicine, number of agent and posts, the medicine writing, footnotes, taste number, prescription costs, post incompatibility in each prescription and use method were investigated. Results In 1 200 prescriptions, involving a total of 230 problems, accounting for 19.2%, mainly including prescription footnotes, pre-prescription, inappropriate diagnosis and medication writing. Conclusion The safety and effectiveness of traditional Chinese medicine prescription is one of the important factor that influence the curative effect of traditional Chinese medicine, it also protects the Chinese medicine clinical medication safety, effective and economic security. Both traditional Chinese physician and Chinese medicine practitioners should be more cautious to select drugs and determine the dosage and usage based on the patient's condition, so as to improve the therapeutic effect and reduce the occurrence of adverse drug reactions.
