双平面胫骨高位截骨在伴下肢力线不良的膝关节后外复合体损伤修复中的作用

摘要：

目的 研究双平面胫骨高位截骨在伴下肢力线不良的膝关节后外复合体（PLC）损伤修复中的作用。方法 将我院56例 PLC损伤患者随机分为研究组和对照组，各28 例。研究组采取以内侧撑开方向的双平面胫骨高位截骨术，对照组采取常规 保守治疗方法，比较两组临床疗效。结果 研究组术后1年下肢机械轴通过胫骨的相对位置、胫骨后倾角高于对照组（P<0.05），胫骨后移距离、外侧间隙张开程度低于对照组（P<0.05）。研究组患者Lysholm 评分优良率明显高于对照组（P<0.05）。两组患 者术后HSS评分及膝关节屈曲度均显著改善（P<0.05），且研究组膝关节屈曲度显著高于对照组（P<0.05）。结论 采取双平面胫骨高位截骨术治疗伴下肢力线不良的PLC损伤，效果理想，膝功能恢复较好，值得推广。

The role of double-plane high tibial osteotomy in the repair of posterolateral complex of knee joint complex with bad lower limb force line

ABSTRACT: Objective To study the role of double-plane high tibial osteotomy in the repair of posterolateral complex (PLC) of knee joint complex with bad lower limb force line. Methods Fifty-six patients with PLC injury were randomly divided into study group and control group, with 28 cases in each group. The study group took open wedge double-plane tibial high osteotomy, and the control group took conventional conservative treatment, the clinical effects of the two groups were compared. Results The relative position of the mechanical axis of the lower limb through the tibia, year after operation and posterior slope anagle in the study group were higher than those of the control group (P<0.05); the distance the tibia moves backwards and degree of lateral clearance opens in the study group were lower than those in the control group (P<0.05). The excellent and good rate of Lysholm score in the study group was significantly higher than that in the control group (P<0.05). The postoperative HSS score and knee flexion significantly improved in the two groups (P<0.05), and the knee flexion in the study group was significantly higher than that in the control group (P<0.05). Conclusion Double-plane high tibial osteotomy has satisfactory postoperative effect in the repair of PLC of knee joint complex with bad lower limb force line, the knee function is better and it is worthy of promotion.
