右美托咪定联合曲马多预防瑞芬太尼麻醉后痛觉过敏的效果研究

摘要：

目的 探究临床手术麻醉过程中联合应用右美托咪定和曲马多对预防瑞芬太尼麻醉后痛觉过敏的效果。方法 将我院从2015年6月至2016年6月收治的78例行上腹部手术治疗的患者随机分为试验组和对照组，每组39例。对照组予以曲马多治疗，试验组予以右美托咪定联合曲马多治疗。观察比较两组苏醒时间、拔管时间、恢复定向力时间及不同时间VAS评分、体征变 化情况。结果 试验组患者苏醒、拔管、恢复定向力时间均显著优于对照组（P<0.05）。试验组的VAS评分和麻醉后体征变化情况（如心率及平均动脉压）均显著优于对照组（P<0.05）。结论 在临床手术麻醉的过程中，联合应用右美托咪定和曲马多可有效预防瑞芬太尼麻醉后痛觉过敏症状，改善患者的VAS评分及各项临床指标，有利于稳定生命体征，值得临床推广使用。

Efficacy of dexmedetomidine combined with tramadol in prevention of hyperalgesia after remifentanil anesthesia

ABSTRACT: Objective To investigate the efficacy of combination of dexmedetomidine and tramadol in the prevention of hyperalgesia after remifentanil anesthesia in the course of clinical anesthesia. Methods A total of 78 cases of patients who underwent elective abdominal surgery in our hospital from June 2015 to June 2016 were selected. The patients were randomly divided into observation group and control group, with 39 cases in each group. Patients in control group were given tramadol, while patients in the observation group were given dexmedetomidine combined with tramadol. The time of awakening, extubation, recovery of directional force, VAS scores and vital signs at different time were observed and compared between the two groups. Results The time of awakening, extubation, recovery of directional force in the observation group were significantly better than those in the control group (P<0.05). Scores of VAS in the observation group were significantly better than the control group (P<0.05). There were significant differences in changes of vital signs (including heart rate and mean artery pressure) between the observation group and the control group (P<0.05). Conclusion The combination of dexmedetomidine and tramadol can prevent hyperalgesia after remifentanil anesthesia and improve the VAS score and clinical indexes in the course of clinical anesthesia. It is helpful to stabilize the vital signs and worthy of promotion and application.
